STEP 2. If you are a NEW credential applicant download the "Credential Application PDF" below; if you are UPGRADING your credentials (from Certified to License, or License to Ordained) please download the "Upgrade of Credentials App PDF."

STEP 3. After you complete the forms, please MAIL your application to:

*You will receive an email from us within 48 hrs to indicate we have received your application. You will then be able to track your application status below at "Credential Status."
